Choo Choo Train Day Care & More, Inc., is a Child Care Center differently and committed to providing our children and their families an excellent service in a safe, fun, educational and full of great love atmosphere.

Our Mission, providing a convenient service according to the needs of working families of Puerto Rico today.

We have all the licenses required both the Department of Family, Health Department and the Fire Department. You must complete the following requirements for admission to enjoy our services: • Original Certificate of Immunization (P-VAC 3) • Medical Certificate by the pediatrician (a) - which provides the pediatrician • Birth Certificate (copy) • Copy of Medical Card Plan • Copy of Social Security • Three (3) 2x2 portraits of children (a)

• Application form duly completed • Copy of ID card of the persons authorized to pick the child • Authorization to provide emergency services duly signed by the parent or guardian • Authorization to supply and administer drugs • Certification signed Reglamente • Authorization to record images / audio and take pictures of activities • Authorization to participate in activities outside the Centre

• Standard Order (is requirement to wear uniforms daily)

• Assessment Interview

• Forms

** Last two weeks of December and two months of summer June and July, the center closes at 6: 30 pm.
