• Physician: Establishes the plan of care for the patient and family. Makes home visits and is available 24 hours a day, 7 days a week.
• Nursing: In conjunction with the physician and other members of the interdisciplinary team, ensures that the patient receives specialized care according to his or her needs. Visits the patient on a regular basis and is available 24 hours a day, 7 days a week.
• Health Aide: Has the responsibility of providing personal grooming care to the patient.
• Social Worker: Assists the patient and family in the social and emotional area.
• Spiritual Counselor: Offers spiritual assistance according to each patient's religious beliefs.
• Dietitian: Evaluates the patient's eating pattern and designs a diet according to the patient's needs.
• Housekeeper: Assists the patient with activities of daily living when family resources are unavailable or untrained.
• Medical Equipment/Supplies: Provides medical equipment and supplies necessary to treat the patient's condition and provide comfort and well-being. Comfort and well-being.
• Medications: All medications related to the condition for which the patient was admitted to hospice will be provided.
Hospice care is a type of medical care that focuses on relieving the symptoms, pain and suffering that certain illnesses can cause a terminally ill patient and on attending to his or her emotional and spiritual needs.
